It is also useful in deciding certain intricacies of treatment including diet and drug tapering too. Hence it can surely be advised in most of our DM cases.
But it is very important to check in following circumstances
1. History of weight loss (irrespective of medicine history)
2. History of DM more than 15 years
3. History of Insulin dependency more than 2 years ( irrespective of BMI/wt loss history)
4. Diabetes is diagnosed before 30 years of age, especially in low to average BMI patients.
but all said C peptide is not a benchmark of Sadhya-Asadhyatva in DM patients and should not be constructed as much. It is always important to give equal importance to clinical history.

First of all congrats on helping the patient reduce his 76 IU of Insulin in such a short interval. Post tapering the sugar levels do fluctuate.
Now you have two options.
I would strictly advise not to restart the insulin injectables again if the compliance from the patient is intact.
Involve the relatives for this task and congratulate the patient and his relatives for what has been achieved and put them on the next goal of further reversal process.

genrally I advise one diet kit( with 40 ml ghee / day ) for first 28 days so as to reverse insuline resistance .
after that patient can be kept on diet chart and monitoring .

Metformin works well in fasting hyperglycemia as it inhibits hepatic glucose production.
as hepatic glucose production is stimulated by stress we need to reduce down stress by advising Pranamyama and yogasana.
Now you need to monitor this patient for perfect execution of guidelines given by you.
The patient should do a minimum 15 min yogasan and pranayama in evening and 30 min in the morning.
in this session Anulom vilom , bhramari and chandranuloma plays a key role.
You should also think of giving small tasks to patients like maintaining records of thoughts.
Along with this morning exercise, use of virechan drugs and medhya rasayana will be helpful to control fasting sugar .
remmber one thing after withdrawing medicines you must educate your patient for such type of sugar fluctuations which is generaly expected .
but in this phase we must remain stable and focus on continious lifestle modifications .
